After receiving multiple warnings in reference to [state basis for eviction, such as non-payment of rent or violation of rules outlined in the lease agreement] and your failure to correct the situation, we have no alternative but to issue this notice of eviction.

The records indicate that [number of months] have passed since your first notice. You have been granted sufficient time to correct the situation. Your continued failure to fulfill your obligations as outlined in the terms of your lease agreement will result in legal proceedings to remove you from the property.
